Referral process - Check List

Diagnosis of appropriate mental health condition (NOT Dementia, Delirium, tobacco use disorder or mental retardation).

Patient discussion and consent for referral signed.

Not actively suicidal – if suicidal, consider ACIS service – ph. 13 14 65.

Preparation of a GP Mental Health Care Plan (Medicare Item #2710).
